Fractional exhaled nitric oxide (FeNO) is produced by airway cells, and its concentration is highly correlated with the number of inflammatory cells, which can be used as a biomarker of airway inflammation. Exhaled nitric oxide concentrations can be detected through mouth breath wound and nasal breath test. The measurement of exhaled nitric oxide is widely used in the diagnosis and monitoring of respiratory diseases.
Inflammatory factors and drugs affect the concentration of FeNO by acting on iNOS (nitric oxide synthase), which increases during inflammation and decreases during anti-inflammation, so FeNO is also widely used as a biomarker of airway inflammation for the diagnosis and monitoring of various respiratory diseases, such as bronchial asthma, chronic cough, COPD and so on.
In 2003, FeNO measurement technology was approved by the US FDA for clinical application; In 2005, Technical guidelines for FeNO testing were published by American Thoracic Society (ATS) and European Respiratory Society (ERS). Later, ATS and ERS clarified the value, scope and testing methods of FeNO in clinical applications, then published FeNO clinical guidelines in 2011. In 2017, ERS developed technical standards for exhaled biomarkers of lung disease, including FeNO. Chinese Guidelines stated that FeNO can be used as an indicator to assess the level of airway inflammation and asthma control, and to predict the response to inhaled hormone therapy in 2020.
